Health and Fitness : Dental Screening reminder for 9th grader

This is a reminder that all 9th graders are required by the Iowa Department of Public Health to have a Dental Screening.

Please visit your Dentist and have them fill out & sign a Certificate of Dental Screening Form. Your dentist should have these forms or have your student pick one up in the office.

Forms should be returned to Deb H. at the High School main office by Friday, May 3rd. The information can be from your child's last dental appointment.

Our audit is scheduled for Monday, May 6th.
